Role: Care Coordinator (Bengali Speaking)

We are hiring Bengali-speaking Care Coordinators to support US-based patients with chronic conditions such as diabetes, hypertension, and heart disease.

This is a 100% remote role from US, ideal for candidates with experience in:

Medical Assistant | Telehealth | Healthcare Coordination | Patient Support | Nursing | BPO Healthcare Process

🎯 Key Responsibilities

  • Conduct monthly patient outreach calls (US patients – Chronic Care Management)
  • Communicate fluently in Bengali + English with patients and caregivers
  • Update EMR/EHR systems (Elation, Athena, PCC, etc.)
  • Review medications, vitals, and care plans
  • Track labs, screenings, follow-ups, and care gaps
  • Coordinate with providers, nurses, and care teams
  • Ensure CMS-compliant documentation and time tracking
  • Identify patient risks and escalate when required
Requirements
  • Fluent in Bengali + English (MANDATORY)
  • 1–3 years experience in Healthcare / Telehealth / Medical Assistant / Care Coordinator / BPO Healthcare
  • Experience with EMR/EHR systems
  • Strong communication + patient handling skills
  • Comfortable working with elderly / chronic care patients
  • Knowledge of HIPAA / healthcare compliance
  • Degree or certification in Healthcare / Nursing / Medical Assistant / Allied Health (preferred)
