The Department is charged with assisting individuals in obtaining needed home and community-based care and services to prevent premature admission into a long-term care facility. The goal of the departments programs is to assist individuals with remaining in their own home for as long as possible. The Department is currently seeking a high energy, self-motivated individual with the desire to assist the Division in supporting the departments goals.<br><br> Responsibilities and duties include: <ul><li>Perform research and analysis of information for the senior nutrition program. </li><li> Provide coordination and professional support for the community-based programs administered by the Division for Aging and Physical disabilities. </li><li> Collaborate with identified providers and community partners to promote the mission and vision of the Department. </li><li> Make recommendations for program policy and regulatory updates and improvements. </li><li> Review the implementation and effectiveness of programs administered by the Division. </li><li> Conduct monitoring and provide technical assistance to providers and participants to ensure timely completion of program requirements; actively engages in collaborative meetings with state providers.
