Job Responsibilities: As a vital member of our integrated care team, you'll leverage your clinical expertise to deliver exceptional primary care. This role offers the opportunity to make a significant impact while meeting established performance benchmarks.

Key Responsibilities:

· Provide comprehensive, patient-centered primary care to adult populations.

· Conduct thorough health assessments, diagnose and manage acute and chronic conditions.

· Perform physical examinations, medication evaluations, wound care, and preventive screenings.

· Utilize performance metrics to guide and enhance patient care strategies.
Qualifications: o Graduation from an accredited advanced practice program.

o Current, unrestricted state license and DEA registration.

o Experience in outpatient primary care would be a plus but New Graduates will be considered.
o Proficient communication skills

o Strong diagnostic and clinical skills.

o Proficiency with Electronic Medical Records (EMR) systems.

o BLS/ACLS certification.
Working Hours: M-F 8pm -5 pm Outpatient
