The successful candidate will be responsible for coordinating and scheduling patient admissions, visits, and services, ensuring seamless delivery of care to our patients and their families.</p> <p><strong>Availability: Full-Time | Open to Remote Work<br></strong></p> <p><strong>Job Responsibilities:</strong></p> <ul> <li>Schedule patient admissions with Admission nurse; F2F visits with Nurse Practitioner or Hospice Physician </li> <li>Manage and coordinate schedules for interdisciplinary team visits (IDG), including physicians, nurses, social workers, spiritual care providers and ancillary staff.</li> <li>Communicate effectively with patients, families, and healthcare professionals to ensure smooth transitions and continuity of care.</li> <li>Maintain accurate and up-to-date records of patient schedules, visits, and services.</li> <li>Collaborate with the admissions/intake team to ensure timely and efficient patient intake processes.</li> <li>Troubleshoot scheduling conflicts and resolve issues promptly.</li> <li>Provide administrative support to the admissions/intake team as needed.</li> <li>Be on top of all PTO or time off request by staff and inform team</li> <li>Inform facilities, families of staff schedules, any changes via email, calls or faxes or a combination of. </li> <li>Assist with plotting orientation schedule for new hires, any scheduled CPR classes, any required or highly encouraged meetings, team meeting schedules</li> <li>Other projects as directed by a manager</li> </ul> <p><strong>Minimum Qualifications:</strong></p> <ul> <li>High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.</li> <li>1-2 years of experience in healthcare scheduling or a related field.</li> <li>Excellent communication and organizational skills.</li> <li>Ability to work in a fast-paced environment and prioritize multiple tasks.</li> <li>Compassionate and empathetic approach to working with patients and families.</li> <li>Proficiency in electronic medical records (EMRs) and scheduling software.</li> <li>Familiar with the geographical location of different counties we serve: San Francisco, San Mateo, Alameda and Contra Costa.</li> </ul> <p>Preferred Qualifications:</p> <ul> <li>Experience working in hospice or palliative care.</li> <li>Certification in medical scheduling or a related field.</li> <li>Knowledge of healthcare regulations and compliance.</li> </ul>
