Monday - Friday 8AM - 4PM

Hourly Pay 40/hr

Job Overview:

Responsible for planning, organizing and carrying out nursing services according to the ADH Center policies and procedures, assuming responsibility for participant and staff activities on an assigned team. Clinical supervision and oversight provided by the Director of Program Development.

Role Expectations:

Primary functions in this role:
  • Provision and supervision of all nursing services
  • Coordinate the completion of, and ensuring the accuracy of, each participant's assessment
  • Coordinate the development, ongoing review, and revision of each participant's plan of care
  • Ensure that nursing notes are written at least one time per month or more often as necessary to reflect each participant's health care status
  • Respond appropriately to any acute changes in a participant's health or functioning
  • Provide appropriate health-care education for each participant and legally authorized representative
  • Assist as necessary in the delivery of other Services
  • Ensure ongoing education, training, and professional development of all Program staff that is relevant to care of participants
  • Obtain, review, and implement primary care provider orders
  • Coordinate the participant's care with the primary care provider and, if applicable, community service providers
  • Supervise medication management services
  • Other duties as assigned by supervisory staff

Qualifications & Skills:

  • Must pass required employee physical before employment begins
  • Current RN license in MA, which must remain current throughout employment
  • Maintain CPR and First Aid certification
  • Minimum 2 years' experience in a community nursing position, preferably working with the elderly
  • Computer proficiency required
  • Must pass criminal background check
  • ​Access to reliable transportation required
Physical requirements form:

Medium Work - Lifting 50 lbs. maximum with frequent lifting and/or carrying of objects weighing up to 25 lbs.

Benefits include the following: VACATION:

Up to 5 years 120 hours per year, or prorated for part-time employees,

After 5 years 160 hours per year, or prorated for part-time employees,

SICK:

Full-time employees may accumulate and carry over from one calendar year to the next a maximum of 240 hours of sick time. Part-time employees working a minimum of 24 hours per week may accumulate and carry over an equivalent pro[1]rated amount.

HOLIDAYS:

6 Closed holidays; 6 Floating holidays

INSURANCE:

Life – employees who have worked at least 6 months are enrolled in the term life insurance policy, premiums paid by the agency.

Medical – full time staff are eligible to enroll in the medical plan at 3 months of employment. CESI pays for 72% of the premium. CESI offers a Health Reimbursement Account for all employees enrolled in the medical plan:

In order to offset potential out-of-pocket costs for employees, Cooperative Elder Services, Inc. will continue to sponsor a Health Reimbursement Arrangement (HRA) in conjunction with the medical insurance plan. Once the plan subscriber pays the first 25% of the deductible out-of-pocket, Cooperative Elder Services, Inc. will reimburse participants for the remaining 75%.

Dental:

Full-time and Part-time employees who were hired and scheduled to work 20 hours or more per week and who have completed at least three (3) months of employment are eligible for participation in the agency group dental insurance benefit. Cost sharing is 50%/50% employee and employer.

Retirement Plan:
